Most families reach us in the middle of something urgent — a hospital discharge with a 48-hour window, a parent who stopped taking medications, a caregiver who can no longer lift safely. You are being asked to choose a home for someone you love, often with no background in senior care and no idea which of the hundreds of licensed communities in Southern California would actually take your parent’s situation seriously.

That is the problem we solve. Golden Pathways is a senior placement and referral service in Pasadena. We tour communities, we know which ones accept two-person transfers or insulin management, and we do the legwork so you are choosing between three good options instead of drowning in sixty listings.

We start with a conversation, by phone or in person, usually 30 to 45 minutes. We ask about daily living needs — bathing, dressing, mobility, incontinence, medication management — along with cognitive changes, behaviors, and any recent falls or hospitalizations. We also talk frankly about money: monthly budget, whether long-term care insurance or VA Aid and Attendance is in play, and whether Medi-Cal or the Assisted Living Waiver may eventually be needed. Location matters too, so we note which family members will visit and how far they can reasonably drive.

You do not get an automated list. A Golden Pathways advisor reviews your assessment against communities we have personally visited and vetted, then calls to verify current availability, real pricing at your parent's care level, and whether the staff can handle the specific needs you described. We check California Department of Social Services licensing records and recent inspection history. Communities that cannot meet the need are removed before you ever hear their names.

We Help You Decide

We schedule the tours, and when families want it, we come along. Knowing what to ask changes everything: caregiver-to-resident ratios on the night shift, how care-level increases are assessed and billed, what triggers a 30-day notice. Once you choose, we stay with you through the physician's report, the admission agreement, and move-in day itself. Families who work with us often keep our number long after placement, and we answer.

You receive a curated shortlist — typically three to five communities — with pricing broken down so you can see base rent versus care-level charges and one-time fees. We explain why each one made the list and what its limitations are. If a memory care community has an excellent activities program but a nurse only on weekdays, you will know that before you tour, not after you sign.

Assisted Living

Licensed communities for older adults who need help with daily activities — bathing, dressing, medication reminders, transfers — but who benefit from a social environment with dining, transportation, and organized programming. Typically 50 to 150 residents.

Memory Care

Memory Care

Secured neighborhoods built for Alzheimer’s disease and other dementias, with staff trained in redirection and behavior management, controlled exits, and routines designed to reduce agitation. We pay particular attention to whether a community will keep a resident as the disease progresses or discharge them when behaviors escalate.

Board and Care Homes (RCHs)

Board and Care Homes (RCHs)

Residential homes licensed for six residents or fewer, in ordinary neighborhoods. The caregiver-to-resident ratio is the draw — often one caregiver to three residents — which suits seniors who are quiet, frail, need hands-on assistance, or would be overwhelmed by a large building. Frequently the most affordable option in Los Angeles County.

In-Home Care

In-Home Care

When your parent is safest staying put, we connect you with licensed in-home care agencies for hourly or live-in support: personal care, meal preparation, transportation to appointments, and overnight coverage.

Hospice Care

Hospice Care

For families facing a terminal diagnosis, we refer to hospice providers who deliver comfort-focused care wherever your loved one lives — at home, in a board and care, or in assisted living — and we help identify placements willing to accept a resident already on hospice.
