Who Qualifies for Hospice Care, and When Is the Right Time?
To qualify for the Medicare hospice benefit, a patient must be eligible for Medicare Part A, have a physician certify a prognosis of six months or less if the illness follows its expected course, and elect comfort care over curative treatment. Two physicians — usually the attending doctor and the hospice medical director — sign the initial certification.
The six-month figure is a prognosis, not a deadline. Care is authorized in benefit periods: two 90-day periods, then unlimited 60-day periods, each requiring recertification. Many patients remain on hospice far longer than six months.
Signs that the timing is right may include:
- ✓Hospice serves any terminal diagnosis — advanced cancer, heart failure, COPD, kidney or liver failure, ALS, stroke, and late-stage dementia.
- ✓Repeated hospitalizations or ER visits,
- ✓Significant unintended weight loss
- ✓Increasing pain, shortness of breath, or weakness
- ✓More time spent sleeping or in bed
- ✓Recurring infections
- ✓Declining ability to walk, dress, or eat without help, and a decision to stop pursuing aggressive treatment.
The most common regret families express is waiting too long. Patients referred in the final days receive a fraction of the benefit available to those referred months earlier.
What Services Does Hospice Care Provide?
Nursing Support
Regular visits from a nurse case manager, plus 24/7 on-call nursing support
Physician Coordination
Medical oversight and coordination with the patient's own doctor
Personal Care Assistance
Home health aide visits for bathing, grooming, and personal care
Medication & Symptom Management
Prescription medications related to the terminal diagnosis, including pain and symptom management
Medical Equipment & Supplies
Durable medical equipment — hospital bed, wheelchair, oxygen, commode — and supplies
Social & Practical Support
Medical social work, counseling, and help with advance directives and planning
Spiritual Care Support
Chaplain and spiritual support, matched to the family's beliefs
Therapy & Wellness Services
Physical, occupational, and speech therapy, and dietary counseling, as needed
Companion Care
Trained volunteers for companionship and caregiver relief
Respite Care
Up to five consecutive days of inpatient respite care so the family caregiver can rest
Crisis Care
Continuous or general inpatient care during a crisis when symptoms cannot be controlled at home
Bereavement Support
Bereavement support for the family for 13 months after the death
Who Pays for Hospice Care?
Most hospice care in the United States is paid for by the Medicare Hospice Benefit under Part A, and patients pay almost nothing. There is no deductible for hospice services. Out-of-pocket costs are limited to a copayment of up to $5 per prescription for pain and symptom management and 5% of the Medicare-approved amount for inpatient respite care, capped at the annual inpatient hospital deductible. The benefit applies even to patients enrolled in a Medicare Advantage plan.
Medi-Cal offers an equivalent hospice benefit in California. Veterans Affairs covers hospice for enrolled veterans, and most private insurance plans and HMOs include a comparable benefit.
One critical exception: hospice does not pay room and board. If your loved one lives in a board and care home, assisted living, memory care, or a skilled nursing facility, the monthly rent and care fee remains the family's responsibility. Hospice layers clinical services on top of that housing at no additional charge.
