When Is the Right Time for Senior In-Home Care?

What Services Does Senior In-Home Care Provide?
Personal care
Bathing, showering, grooming, dressing, toileting and incontinence care, transfers, mobility assistance, and fall prevention.
Household support
Meal planning and cooking, light housekeeping, laundry, changing linens, and keeping walkways clear.
Health-related support
Medication reminders, monitoring for changes in condition, accompaniment to medical appointments, and communication with family and physicians.
Errands and transportation
Grocery shopping, pharmacy pickups, and transport to appointments, church, or social events.
Companionship
Conversation, games, walks, hobbies, and help staying connected with family — often the part families undervalue and seniors value most.
Specialized care
Dementia and Alzheimer's support, post-hospital and post-surgical recovery, respite for family caregivers, and end-of-life support alongside a hospice team.
Skilled Nursing Care
Caregivers cannot administer injections, change sterile dressings, or perform other skilled nursing tasks. Those require a licensed nurse through a home health agency.
Who Pays for Senior In-Home Care?
Most in-home care is private pay. In the Los Angeles area, agency rates generally run from about $35 to $45 per hour; week costs roughly $3,000 to $3,900 a month. Around-the-clock care typically exceeds $20,000 a month — the point at which an assisted living, board and care, or memory care becomes a more affordable option. Funding sources include:
Private funds
Savings, pensions, Social Security, family contributions, or a reverse mortgage
Long Term care insurance
Most policies cover licensed in-home care once a benefit trigger is met
IHSS (In-Home Supportive Services)
A county-administered Medi-Cal program that pays a caregiver, often a family member, for eligible low-income seniors. The county assesses authorized hours
Medi-Cal managed care
Some plans now offer personal care and homemaker services as a Community Support under CalAIM
VA Aid and Attendance
A monthly pension benefit for qualifying wartime veterans and surviving spouses, plus VA homemaker and home health aide programs
Medicare
Pays for intermittent skilled home health, not ongoing custodial care. Some Medicare Advantage plans include limited in-home support benefits.
Medicare does not pay for board and care. It covers short-term skilled nursing and rehabilitation after a qualifying hospital stay, not long-term residential care.
